Fees and Insurance

  • Reduced Fee Policy for Self-Pay Clients

    A discounted rate is available for clients who either do not have insurance or have chosen to opt out of using their insurance and pay the full session fee on the day of the appointment.

  • Important Information About Using Insurance for Therapy Services

    When you choose to use insurance for therapy, your therapist is required to provide a clinical diagnosis, which becomes part of your medical record. This diagnosis is based on your current symptoms, even if you have received a different diagnosis in the past. If your symptoms do not meet the criteria for a diagnosis, insurance cannot be used to cover your sessions. You can, however, choose to continue services through privately paying for services.

    Your therapist will review and discuss the diagnosis with you. Please be aware that your insurance provider will have access to your diagnosis, as well as the dates and times of your sessions. In some cases, they may also request additional details in order to approve continued treatment.

    If you are not on an insurance plan and/or prefer not to use insurance, you have the option to pay out-of-pocket. To do so, please let me know you’d like to opt out of insurance billing.

    Session Payment Policy

    Payment for sessions is expected at the time of your appointment unless a different arrangement has been made in advance or your insurance plan specifies otherwise. Copayments must be paid at the time services are provided.

    If you plan to use insurance, you must provide Combat Mental Health with accurate and up-to-date insurance details, including a copy of your insurance card. It is your responsibility to verify what mental health services your insurance plan covers. While we are available to help you navigate this process, you are ultimately accountable for understanding your coverage. You are also financially responsible for any portion of the cost not covered by your insurance provider, such as copays, deductibles, or coinsurance.

  • Notice of Your Right to a Good Faith Estimate

    You are entitled to receive a Good Faith Estimate that outlines the anticipated cost of your mental health care.

    In accordance with federal law, healthcare providers must provide an estimate of expected charges to individuals who either do not have insurance or choose not to use their insurance for services. This applies to all medical and mental health services, including psychotherapy.

    You have the right to request a Good Faith Estimate from your provider or any other provider you’re considering, before scheduling a service.

About Heather

My name is Heather Pererva, MS, LMFT, and I am the owner and sole provider of Combat Mental Health. I am a Licensed Marriage and Family Therapist with a Bachelor of Science in Community Psychology and a Master of Science in Marriage and Family Therapy from St. Cloud State University. My approach to therapy is rooted in Systems and Biopsychosocial Theories, and I utilize an integrative, person-centered style that is tailored to meet the unique needs of each client.

I strongly believe that while our past shapes our present, it does not have to define our future. Our experiences hold value—offering both the survival strategies that once protected us and the life lessons that guide growth and change. We are not bound by what has been; we each have the power to choose a different path forward. I recognize the strength it takes to enter therapy and am dedicated to creating a supportive, nonjudgmental space where clients feel safe to be themselves, learn to accept, understand, and overcome their past, and feel empowered to pursue their personal goals.
